TERMS & CONDITIONS
Participant Release of Liability
The risk of injury from the activities involved in this program are significant. These include the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown.
EVEN IF ARISING FROM THE NEGLIGENCE of others, I assume full responsibility for my participation and I am willingly ready to comply with the stated and customary terms and conditions for participation. If however I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such hazards to the attention of the nearest staff member immediately. At the same point, I for myself and on behalf of my heirs, and any personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE 24 HOUR GYM, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (RELEASEES). WITH RESPECT TO ANY AND ALL INJURIES, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. INITIALS – – – – –
I the member grants The 24 Hour Gym of Budd Lake, it’s representatives and agents the authorization of the sending of communication via e- mail, including late payment notifications, solicitations and other correspondence.
This contract shall not obligate the buyer for more than 12 months from the date the contract is signed.
NOTICE TO THE CUSTOMER
You are entitled to a copy of this contract at the time you sign it. You may cancel this contract at any time. If you choose to cancel, you must either:
Send a signed and dated written notice of cancellation by registered or certified mail, return receipt requested; or,
Personally, deliver a signed and dated written notice of cancellation to: The 24 Hour Gym of Budd Lake, 141 Route 46 East, Budd Lake, NJ 07828; or
Send an email to The24hourgymofbuddlake@gmail.com
All Open-Ended and Month-to-Month memberships are subject to cancellation at any time by the member, with 30 days written notice and the return of the access key.
I understand once my membership becomes open-ended, I must notify The 24 Hour Gym of Budd Lake via written notice at least 30 days prior to the upcoming, monthly payment due date to cancel it.
In the circumstance that a notice of cancellation is given less than 30 days prior to the upcoming monthly payment due date, the member shall be responsible for that monthly payment.
I understand the cancellation policy for my membership at The 24 Hour Gym of Budd Lake hereinafter called COMPANY. I agree it is to remain in full effect until COMPANY has received written notification from me of its termination in such a time and manner as to afford COMPANY a reasonable opportunity to act on it, or until all payments due under the contract have been made.

NOTICE TO BUYER: You are entitled to a copy of this contract at the time you sign it. Keep it to protect your legal rights. Do not sign this contract if blank. I certify that I have received a completed and signed copy of this membership agreement and any other documents which I have signed. This represents the entire agreement between buyer and health club facility. I further certify that I have read both sides of this agreement prior to affixing my signature and understand and agree to all terms and conditions as stated.
